I understand this includes a personal intake survey, a personal insight behavior and motivation report, a full day of workshops and sessions, and a follow up debrief session with Cindy Miller (after the workshop). 

I understand that this payment is nonrefundable. 

I understand my personal debrief session with Cindy Miller needs to be scheduled within six weeks after the completion of the workshop.
